Below is our companies work agreement required before a survey can be performed. The charges for the services performed and the date(s) of of the service will be agreed on between the client and Sun Coast Marine Surveying before this document is printed, signed and initialed. The form can be printed by selecting the “Download” button/icon located at the bottom left side of the document viewer. Click “Open” once it appears on your screen and then click the “Print” icon on the upper left side of the document. Please read the document carefully and initial each page and sign the last page. You can scan and email the agreement to us at: Surveyor@MiamiMarineSurveyorInc.com or fax it to the number on the top of the agreement. If the survey is schedueld with short notice you can give it to the surveyor the day of the survey if you are going to be present. If downloading the form is not possible a surveyor can bring a copy of the form the day of the survey. The completed and signed agreement form and payment for services must be received by the day of the survey (or service) before the services are rendered.
